| 1) Is there a cost to join the program?
There is NO COST to join the program.

| 2) Will the same drugs always be on the list?
Medications may be added or removed from the list at any time.
You can always inquire about a medication prior to getting it filled. You can also
view a current list of drugs that qualify online by
clicking here for an alphabetic list by Generic Name or
clicking here for an alphabetic list by Brand Name

| 3) Can anyone join the program?
Yes, it is open to anyone in the states we serve, whether you have insurance or not. In some cases this
program will be less expensive than your prescription drug co-pay.

| 4) If a drug is on the list, does that mean all strengths of the drug are included?
No, Only the exact strength of the drug listed is eligible for this program.

| 5) I want to pick my prescription up at the pharmacy in
Burlington, What's the Procedure?
Have your Doctor write you a prescription for the drug and quantity you would like. You can
then bring it to the pharmacy and wait for it to be filled (about 15 minutes). Another option
would be to have your Doctor telephone the prescription into the Pharmacy and you
can pick it up at your
convenience. The last option would be to fill out this
online request form
and we will contact your doctor for you and notify you when your prescription
is ready to be picked up. Only use the online request form if you are going to be
picking your prescription up at the pharmacy.

| 6) What states do you currently service?
We are currently accepting orders from
Kentucky, Ohio and Indiana. |

| 7) How do your prices compare to the new Medicare Discount Cards?
Our prices in most cases will be better than the new Medicare Discount Cards being offered!

| 8) How do I get my prescription mailed to me?
a)
Print this form
b) Fill out all of the information requested.
c) Have your doctor write you a prescription for the quantity you would like to have.
d) Mail us the filled out form, the prescription and payment. (We accept Visa,
Mastercard, Discover, checks or money orders)

| 9) How much does shipping cost?
There is a flat $4.00 shipping fee per order. (please note we can mail multiple
prescriptions for a single $4.00 per order fee)

| 10) How long will it take to get my prescription?
Once we receive your prescription and payment information, your order will be
processed within 24 hours and shipped to you. We ship orders Monday Thru Friday
by UPS

| 11) Do you sell other items besides prescriptions?
Yes we do. We are a full line pharmacy with access to over 70,000 items.
